A VA disability form, Social Security award form, or any type of record from a medical center which documents your diagnoses will do. A copy of your diagnoses downloaded from your patient web portal is also acceptable.
If you are not sure if your condition qualifies, then read the next section. We are not at this time able to certify for either glaucoma nor "intractible pain." You will need a specific diagnosis on your record such as fibromyalgia, arthritis, peripheral neuropathy, or other such qualifying condition. See the next section for specific qualifying diagnoses. A medical marijuana doctor may certify you for:
Positive status for human immunodeficiency virus/acquired immune deficiency syndrome
Amyotrophic lateral sclerosis
Post-traumatic stress disorder
Cachexia or wasting syndrome
Intractable pain which is pain that has not responded to ordinary medications, treatment or surgical measures for more than six (6) months
Seizures including, without limitation, those characteristic of epilepsy
Severe and persistent muscle spasms including, without limitation, those characteristic of multiple sclerosis
Any other medical condition or its treatment approved by the Arkansas Department of Health.
Many people prefer more natural treatments than opiate pain medications or benzodiazepines. Alternative medical centers can be helpful in this regard. Marijuana may be a safer alternative compared to other scheduled medications, especially in regard to dependency, withdrawal, and overdose risk.
We are happy to get records for you, but it may be faster for you to get copies for yourself. People do it all the time. Having personal copies of medical records may be helpful for after hours emergencies.
How To Obtain Your Records
Ask for a release of information form. This is available at all clinics and hospitals.
Be sure to list yourself as the recipient of records. Specify that you only need the Initial Evaluation and last one or two follow-up visits, if available.
You do not need the entire record.
In the Reason For Record Request section write down "Personal copies for after hour emergencies or physician communication" as your reason to request records. Everyone should have copies of their medical records including a diagnosis list and medications anyway. You never know when it will be needed.
You are done! You can usually return in a few days to pick them up. There is sometimes a small fee.

We no longer publicise our address here. We have had a couple of people come in unannounced who were disruptive. In addition there have been two occasions where people wandered through the area asking about the "marijuana clinic." We don't wish to be a disruption to neighboring businesses nor have a lot of traffic. This is only a small part of what we do. We have perhaps certified 40 people in the last year. We are happy to provide the address to people who want to arrange an appointment or drop by with questions.
